“Home medical care” promotion department

Wyoming Medicaid, part of the Wyoming Department of Health (WDH), touts the benefits for both patients and providers from what is known as the patient-centered home health (PCMH) care model.
Dr. Paul Johnson, WDH's Wyoming Medicaid Medical Director, encourages patients to seek out clinics that are recognized to offer the PCMH approach. “We have seen this model deliver healthy outcomes, including increased uptake of preventive services and screenings, and help improve care.”
Here are the patient benefits Johnson describes:
- Stay healthy with more preventive care
- Improved access to clinical advice and medical records, and improved communication.
- Better manage chronic diseases
- Gain a better experience with team-based care
To encourage clinics and clinics to offer the PCMH care model, Wyoming Medicaid has established a set of standards that they must meet to obtain certification. “Medical practices in Wyoming that work with us to achieve PCMH designation could see significant benefits,” Johnson said.
Benefits of PCMH certification include:
- Increased reimbursement support from federal, state, and commercial payers
- Increase efficiency and reduce practice costs
- Increased patient perception that clinics provide enhanced services and higher quality care
