The Columbus Regional Health Department is conducting health surveys by phone and online through May.
The information collected in this survey will help CRH identify and address the region's most important local health issues. The survey is conducted by an independent national organization, China, and is completely anonymous. There are two ways to complete the survey.
- The research company PRC calls local residents with the caller ID “Col Regl Health.”

This health status survey has been conducted every three years since 1996. It is a systematic, data-driven approach to determining the health status, behaviors, and needs of residents within Columbus Regional Health Services' service area. The information collected will inform decision-making to improve community health and wellness, ensure adequate and equitable access to health services, and surface health disparities, especially around families with children. and will be used to guide efforts by CRH and the CRH Healthy Communities Initiative.
Participants will not be asked to provide any personally identifying information. This is an anonymous survey.
